The Recovery Support Specialist provides skill building, engagement, and care coordination for patients in recovery at InterCommunity's Primary Care locations. This role involves collaborating with transdisciplinary teams to link clients to clinical, medical, and social services while supporting their overall recovery journey. The specialist also facilitates groups, assists in crisis assessments, and provides education and consultation to clients, families, and the MAT team.
InterCommunity Health Care is a FQHC (Federally Qualified Health Center) that provides primary care, behavioral health care, and addiction recovery services in the Greater Hartford area.
