About this role
Registered Nurse Case Manager responsible for comprehensive assessment, planning, implementation and evaluation of patient needs across the care continuum. The role collaborates with physicians, patients/families and interdisciplinary teams to coordinate post-hospital care, manage length of stay, and ensure regulatory and payer requirements are met. The position serves patients across all age groups and supports discharge planning and transitions to appropriate levels of care.
