About this role
The RN Care Manager at Strive Health supports people with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD) by coordinating comprehensive, patient-centered care across the care continuum. The role is part of the Clinical Care team and focuses on engaging patients and local providers using Strive’s high-touch, data-driven model. This is a hybrid position primarily conducted via telecommunication with occasional in-person visits to clinics or patient homes. The role reports to the Senior Manager, Clinical Care.

Qualifications
- Associate’s Degree in Nursing
- Registered Nurse (RN) License
