RN Care Manager

The RN Care Manager conducts Medicare Annual Wellness Visits and health risk assessments to promote patient wellness and coordinate care across multiple Cherry Health medical sites. This role collaborates with primary care providers, specialists, and the healthcare team to develop individualized care plans and ensure continuity of care for patients transitioning between settings. The position requires adherence to organizational standards and represents the organization's mission in all actions and communications.
