RN Care Manager

The RN Care Manager leads a team of Care Coordinators responsible for case management, health management, and transition of care activities to assist members with their healthcare needs. The role involves providing direction and guidance to ensure alignment with the model of care and regulatory requirements, managing caseloads based on complexity, and maintaining electronic documentation systems. The position also includes promoting multidisciplinary collaboration, auditing case management assessments, and participating in internal and external audits. The ideal candidate will have an active California Registered Nurse License, experience leading care coordinators, and knowledge of managed care principles.
