The Denver Hospice
The Denver Hospice is seeking an RN Case Manager to oversee and coordinate patient and family care in accordance with hospice philosophy. This role involves providing skilled nursing, conducting assessments, and collaborating with the Interdisciplinary Team to develop and evaluate care plans. The position requires daily travel and a current Colorado RN license.
