Care Synergy
The RN Case Manager oversees and coordinates patient and family care in accordance with hospice philosophy, providing skilled nursing, assessments, and supportive care. This role involves collaborating with the Interdisciplinary Team to develop care plans, monitor patient needs, and ensure educational and logistical support for patients and families.
