RN Navigator

The RN Navigator provides clinical navigation and care coordination for patients across Mosaic's ambulatory continuum, serving as a consistent point of contact for high-risk patients with complex needs. The role involves risk-stratified follow-up, seamless transitions among primary care, specialty partners, and community services, and urgent referral management. This position ensures timely follow-up, effective navigation of care, and coordination with internal and external partners to support patient safety and continuity.
