RN/Case Manager-MSH-Case Management-FT-Weekdays/Weekends/Holidays

The Case Manager is responsible for managing all aspects of patient care, including determining the appropriateness of the level of care, developing and implementing care plans, and identifying expected lengths of stay. The role involves collaborating with physicians, social workers, and other interdisciplinary team members to reduce length of stay, improve efficiency, and ensure quality resource utilization. The Case Manager also conducts utilization reviews, manages discharge planning, and handles appeals processes to support timely patient progression through the continuum of care.
