Service Coordinator

The Service Coordinator provides services to eligible individuals to gain access to medical, social, educational, and other needed services across various Medicaid waiver programs. The role involves crisis prevention, assessment, monitoring, and advocacy while maintaining regular contact with individuals, families, and service providers. The position requires extensive travel within Archer, Clay, and Wichita Counties to support adults and children on the assigned caseload.
