Responsibilities

• Assess and support families to identify risks, needs, and strengths, then create case plans that promote stability and respect cultural values.
• Provide crisis intervention, mediation, and conflict resolution, and coordinate case conferences and family meetings to achieve positive outcomes.
• Connect families to community resources, track progress, and adjust plans to ensure the well-being of children and families.
• Manage an active caseload, work closely with families to implement and monitor case plans, and advocate for children and families to ensure access to essential services and supports.
• Develop and maintain strong relationships with community partners and service providers to enhance support for families and lead initiatives that prevent child intervention, strengthen family resilience, and ensure adherence to policies, cultural protocols, and legislation.
