Social Worker

This entry-level Social Worker role provides psychosocial services to in-center and home dialysis patients, collaborating with the interdisciplinary team to promote positive adjustment, rehabilitation, and improved quality of life. The position involves conducting patient assessments, providing counseling, facilitating care planning, and addressing barriers to treatment adherence. The Social Worker also supports staff education on the emotional and psychological impacts of Chronic Kidney Disease and participates in quality improvement activities.
