Social Worker

This role provides social work services to assess and enhance patients' and families' ability to cope with physical, emotional, and environmental problems related to illness and discharge planning. The Social Worker collaborates with the multidisciplinary healthcare team to establish appropriate discharge plans and provides counseling and advocacy for patients and their significant others. The position involves conducting psychosocial assessments, implementing intervention plans, and coordinating with community resources to meet the continuum of care needs. Additionally, the role includes supervising field placement students and mentoring new staff members.
