Social Worker

This role is responsible for ensuring clients receive comprehensive medical care and services to optimize their substance use, mental health, and health outcomes. The position involves conducting social evaluations, assessments, and maintaining participant records while coordinating socio-health resources. The Social Worker also provides guidance on housing and employment, facilitates therapy sessions, and supports client reintegration into the community.
