Social Worker

This entry-level Social Worker role provides psychosocial services to in-center and home dialysis patients, collaborating with the interdisciplinary team to promote positive adjustment, rehabilitation, and improved quality of life. The position involves conducting patient assessments, providing counseling, facilitating care planning, and supporting patients through the transplant referral process and financial assistance programs. The Social Worker also participates in quality improvement activities and ensures comprehensive care for patients with Chronic Kidney Disease.
