About this role

The START Coordinator supports individuals with intellectual/developmental disabilities and co-occurring behavioral health conditions by conducting assessments, coordinating care, and providing crisis intervention. This role involves maintaining an active caseload, collaborating with interdisciplinary teams, and ensuring fidelity to the START model of service. The position requires regular outreach, documentation, and on-call support to optimize independence and community living for clients.

Responsibilities

Conduct in-person and remote assessments and evaluations for individuals with intellectual and developmental disabilities
Coordinate interdisciplinary team meetings and develop comprehensive crisis prevention and intervention plans
Maintain active caseload and provide systemic consultation using ecomaps and START model best practices

Skills

Qualifications

Master's degree in Mental Health, Psychology, Counseling, Social Work or other Human Service fieldSTART Coordinator Certification
