START Coordinator

The START Coordinator provides comprehensive case management, crisis support, and coordination of care for individuals at-risk for acute crisis or experiencing behavioral health concerns. This role involves training and supervising therapeutic coaches, developing intervention plans using the START service model, and collaborating with multi-disciplinary teams to ensure high-quality services. The position requires maintaining an active caseload, conducting outreach visits, and participating in on-call support rotations.
