Stroke Program Nurse Navigator

The Stroke Program Nurse Navigator provides expert nursing consultation and practice oversight as a liaison between medical staff, nursing staff, family members, and support services. This role coordinates multidisciplinary care based on evidence-based practice, delivers stroke education, and oversees patient care including post-discharge follow-up. The position collaborates with key stakeholders to meet stroke program certification requirements and incorporates the patient experience into all aspects of care.
