Suicide Prevention Case Manager

The Suicide Prevention Case Manager works with Veterans and their families to identify needs, develop individualized service plans, and connect participants with resources that support their safety, stability, and overall well-being. As part of the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program, the Case Manager provides ongoing support to Veterans at risk of suicide, helping address challenges related to housing, employment, financial stability, mental health, and social isolation. The role combines direct case management, field-based services, staff supervision, and documentation while collaborating with clinical social workers, peer specialists, and community partners.
