WestCare
The Case Manager provides screening, assessment, case management, care coordination, referrals, and supportive services to Veterans and eligible Service Members participating in the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program. Working collaboratively with participants, the Case Manager promotes safety, stability, wellness, and access to VA and community resources through a Veteran-centered, strengths-based, and trauma-informed approach.
