Support Assistant (Days)

Support Assistants work with adults who have recently been discharged from hospital or an integrated care facility, supporting them in their own homes to regain confidence and independence through personal care, medication assistance, and daily living support. The role is part of the Supporting Independence Team and plays a vital role in the discharge to assess model, helping individuals recover in the right environment and assess their longer-term needs. This position requires flexibility and reliability to travel across Hull, ensuring timely and effective support for vulnerable adults.
