Supports Case Worker

Venture Community Services is seeking a Supports Case Worker to provide person-centered case management, coaching, and coordination to individuals with disabilities through Supported Collaborative Living and Independent Living Supports programs. The role focuses on helping individuals maintain independence in their homes and communities while ensuring necessary supports, services, and documentation are properly managed. This position involves building professional relationships, assisting with daily living skills, financial management, healthcare coordination, and community resource access.
