Targeted Case Manager

COMHAR is seeking a Targeted Case Manager to work directly with adults, including young adults and members of the Hispanic community, to assess needs, develop service plans, and coordinate community-based supports. The role involves meeting clients in various settings to provide hands-on guidance, advocacy, and crisis management to help them navigate services and achieve greater stability and independence. This full-time position is based in the Philadelphia, PA area with a Monday-Friday schedule.
