As a Home Stretch Transition Coach Case Manager, you will provide individualized support and case coordination to young people transitioning from Out-of-Home Care to independence. This role involves developing natural networks, brokering financial resources, and collaborating with stakeholders like foster families and community members to ensure stability and self-determination for youth up to age 21.
