Transitional Care Coordinator - PRN

About this role
Supports the Case Management team by assisting RN Case Managers and Social Workers with discharge planning, placement coordination, insurance communication, and data collection. Works with multidisciplinary care teams to facilitate patient care transitions across the continuum and follows identified patients post-discharge. Engages with patients across the lifespan and provides both on-site and remote coverage as needed.
