Transitional Care RN

As a Nurse Coordinator at MJHS, you will act as a patient advocate to facilitate and coordinate transitions to the next level of care, aligning with applicable regulations and agency policies. You will collaborate with MJHC and inpatient facilities to evaluate patients referred for transitional planning or home health services through telephonic outreach and team efforts. This role requires working closely with discharge planners, MJHC, and case management staff to ensure smooth care transitions.
