Veteran Care Coordinator

The Veteran Care Coordinator provides direct, in-person case management services to vulnerable Veterans and their households who are homeless or at risk of homelessness. This role focuses on supporting housing stability through trauma-informed, tailored services and collaboration with a team under the VA Supportive Services for Veteran Families program. The position involves regular client contact, documentation, and occasional travel throughout Colorado.
