Waiver Case Manager (DD)

This role provides case management services to adults accessing waiver and state plan services, coordinating medical, social, educational, and other supportive services to meet their basic and vocational needs. The case manager maintains a caseload of no more than 50 individuals, assessing changing needs and linking them to appropriate community resources. The position involves collaborating with families, service providers, and supervisors to develop and monitor effective service plans.
