The Wellness Coach role involves building relationships with customers to support seniors living independently at home through wellness coaching and care insights for family caregivers. The position utilizes Remote Patient Monitoring (RPM) and sensor data to drive proactive and reactive communications, ensuring ongoing independence and quality of life for seniors. The role requires analyzing customer profiles, making sound decisions in emergencies, and managing communications via phone, email, and chat. Wellness Coaches are expected to establish trusted relationships, meet performance metrics, and contribute to process improvements while maintaining confidentiality and professionalism.
