Wraparound Facilitator

The Wraparound Facilitator develops and coordinates strength-based, family-centered teams to meet the individualized needs of children and families in accordance with the Wraparound Model. This role involves providing assessment, service planning, advocacy, and life skills training to help families achieve independence and self-sufficiency. The position emphasizes culturally competent service delivery and adherence to the Wraparound Model's fidelity standards.
