Wraparound Facilitator

This role provides ongoing intensive case management services through Wraparound facilitation and the possible use of the YES Waiver for youth with mental illness aged 3-18. The position involves coordinating services, conducting assessments, and collaborating with schools, juvenile probation, and other agencies to support youth and their families. The Wraparound Facilitator is responsible for planning and implementing individualized care plans while maintaining flexibility to address crises and solve problems independently.
