Medical Home Network (MHN) is a Chicago-based public benefit corporation that partners with federally qualified health centers to transform care in the safety net and build healthier communities. It combines care management technology (MHNConnect) with participation in value-based payment models and ACO programs (ACO REACH, MSSP) to drive better outcomes, quality, and cost savings for Medicaid, Medicare, and uninsured populations.
