Bachelor-Level Care Manager

Paragon Behavioral Health Connections is seeking a Bachelor-Level Care Manager to provide intensive community-based care management services within the Community Access & Partnership (CAP) Program, with a dedicated focus on supporting residents of Mercy Housing communities. The Care Manager plays a critical role in helping clients achieve psychiatric stability, maintain housing, reduce crisis utilization, improve independent living skills, and successfully transition toward less intensive levels of care. This position requires extensive field-based work, community outreach, and collaboration with internal and external providers throughout the Denver Metro area and surrounding communities, including regular on-site presence at Mercy Housing properties.
