About this role
The Bilingual Member Services Representative / Lead Case Manager supports Pair Team’s whole-person, interdisciplinary care model by engaging individuals with high clinical and social needs and building trusting relationships to improve access to care. This full-time role is focused on outreach and member engagement within an Enhanced Care Management team and partners closely with clinical staff. The position is primarily remote with regular community-based on-site visits 2-3 times per month.
