The RN Care Manager is a telephonic, remote nurse based in Puerto Rico who uses clinical knowledge to assess and evaluate members' needs and support their ongoing wellness. The role guides members with chronic conditions to appropriate resources and coordinates care while working with minimal direction and reporting to a Manager of Care Management. Bilingual proficiency in English and Spanish is required.
