Care Coordinator

The Care Coordinator role at Integrated Services for Behavioral Health supports youth and families in Southeastern and Central Ohio through the OhioRISE Medicaid managed care program. This position involves creating and facilitating collaborative care teams to identify needs, set goals, and connect individuals with essential behavioral health and community resources. The coordinator works directly with families in their homes and communities to promote well-being and reduce hospitalizations while ensuring client voice and choice in all care decisions.
