Care Manager RN or LPN

The Care Manager collaborates with patients, families, and healthcare team members to promote a patient-centered model of care. The role involves assessing, planning, coordinating, and evaluating patient needs to prevent disease exacerbation, improve health outcomes, and minimize hospital utilization. The Care Manager also provides education, supports self-care management, and coordinates care transitions.
