Case Manager

Mission Australia is seeking a passionate Case Manager to deliver intensive, strengths-based support to families experiencing complex challenges within the Families Together Program. The role involves conducting home visits, assessing family needs, developing tailored case plans, and coordinating with various services to prevent child protection interventions. By providing holistic, in-home assistance, the Case Manager works collaboratively to strengthen parenting capacity, ensure child safety, and create long-term positive outcomes for vulnerable families.
