Case Manager

The Case Manager provides housing-focused case management and stabilization services to individuals experiencing homelessness, utilizing Housing First, trauma-informed, harm-reduction, motivational interviewing, and strengths-based approaches. The role involves maintaining a caseload of about 25 participants, conducting assessments, developing service plans, coordinating resources, and collaborating with internal programs and community partners to achieve stable housing.
