Case Manager

The Case Manager conducts screenings, assessments, and counseling to coordinate housing placements for homeless individuals with histories of mental illness, substance use, and/or HIV/AIDS. This role involves creating individualized treatment plans, facilitating support groups, and assisting clients in obtaining necessary documentation for benefits and permanent housing. The position requires collaboration with internal departments and referral agencies to ensure quality care and successful reentry into the community.
