Case Manager

This position serves as the primary consumer advocate, coordinating all services delivered to consumers and their families both internally and through external referrals. The role involves conducting needs assessments, developing service plans, and providing case management sessions to support consumer well-being and recovery. The incumbent is responsible for documenting clinical activities, preparing progress reports, and coordinating care with treatment teams and external providers.
