Case Manager

This role serves as the primary consumer advocate, coordinating all internal and external services delivered to consumers and their families. The position involves conducting needs assessments, developing and reviewing service plans, and documenting clinical activities in compliance with licensing and regulatory standards. The Case Manager also facilitates referrals, coordinates care with treatment teams, and prepares progress reports for courts and other agencies.
