Community Services Case Manager

A Community Services Case Manager at St. Mary's Center provides comprehensive AAA-aligned case management and Medi-Cal administrative activities to older adults and vulnerable populations in Oakland and Alameda County. The role involves conducting biopsychosocial assessments, developing individualized care plans, coordinating medical and social services, and advocating for benefits to ensure safe housing and wellness. You will maintain an active caseload of approximately 30 participants while collaborating with multidisciplinary teams, community partners, and family caregivers to improve social outcomes and prevent crisis.
