CSS Case Worker

The CSS Case Worker provides community support and case management services to address the social determinants of health for patients at APLA Health Center locations. The role involves assessing patient needs, connecting individuals to health and community resources, and maintaining detailed records within the Electronic Health Record system. This position requires collaboration with multidisciplinary teams and external stakeholders to empower patients in managing chronic conditions and improving their social drivers of health.
