Social Worker

The Social Worker provides clinical social work services and biopsychosocial assessments to meet the diverse needs of patients at APLA Health Center locations. This role involves collaborating with medical and behavioral health staff to develop treatment plans, coordinate community resource referrals, and conduct crisis interventions. The position requires maintaining a caseload of 40-60 patients and documenting all interactions within the Electronic Health Record.
