Southwestern Ontario Aboriginal Health Access Centre
The Indigenous Patient Navigator at SOAHAC serves as a trusted support to Indigenous individuals and families, helping them navigate hospital and community health systems. Grounded in Indigenous values and employing a Two-Eyed Seeing approach, the role bridges traditional healing practices with Western clinical care to ensure culturally safe transitions and equitable outcomes. The position involves coordinating discharge planning, advocating for patient rights, and collaborating with diverse stakeholders including Elders, physicians, and community organizations.
We provide high quality, wholistic health and wellness services throughout Southwestern Ontario. Services are provided to Indigenous (First Nation, Métis, Inuit) people and their families, living on and off-reserve, in rural areas, and in the urban Indigenous communities in London, Owen Sound, Newbury, Waterloo Wellington and Windsor.
