MDS Coordinator (MDS Experience required)

The MDS Coordinator at Avamere Rehab of Cascade Park is responsible for planning, scheduling, and revising the Minimum Data Set (MDS), including implementation of CAA’s and triggers. The role involves reviewing patient care plans, coordinating care plan conferences, managing utilization review, and ensuring compliance with regulatory assessments in an 88‑bed skilled nursing community.
