MDS Coordinator

The MDS Coordinator conducts and coordinates the completion of required Resident Assessment Instruments (RAI) and is responsible for implementing and evaluating each resident's comprehensive plan of care. This role involves managing the facility's RAI process, ensuring accurate MDS assessments, and leading interdisciplinary care plan team meetings. The position serves as a primary resource for problem-solving regarding the skilled nursing facility prospective payment system and quality reporting program.
