This center-based Medical Assistant role supports Habitat Health’s PACE participants by assisting the interdisciplinary team to promote participant well-being and continuity of care. The position helps enable high-quality, coordinated clinical and social care for older adults, documents participant information in the EMR, and contributes to a welcoming center environment. The role is mission-driven and focused on serving frail or high-risk seniors in collaboration with clinical partners.
