About this role
Strive Health is hiring a Patient Navigator to help engage and support patients with kidney conditions in Strive’s coordinated care program. The role focuses on building patient relationships primarily through phone interactions, with some in-person engagement at clinics or patient homes within an assigned region. This position reports to the Manager, Patient Growth and supports program growth through patient enrollment and ongoing engagement.
